Admission Requirements
The State of Indiana has established requirements for admission
to a health care and rehabilitation facility such as Morristown
Manor. When being discharged from a hospital, transferred from another
health care facility, or coming directly from a person’s own home,
the state requires the following records to be received by the provider
of health care:
1. An evaluation of the prospective resident to ensure the facility
will meet the medical, psychological, and social needs of the individual.
2. A medical history and physical examination report by the resident’s
physician, completed not more than 30 days prior to admission, and
provided to the facility upon admission or within 48 hours after
admission.
3. A chest x-ray completed within six months of admission or at
admission.
4. A Mantoux (tuberculin skin test) completed within three months
prior to admission or administered upon admission and read 72 hours
after admission.
5. Admission or transfer orders signed by the attending physician,
with current medication (prescription) orders, and any other special
orders for care.
6. Agreement of the physician to visit the resident by State rules
or by the condition of the resident. If your physician is unable
to visit the resident here, our staff and Medical Director will
arrange for physician coverage.
7. Pre-admission screening completed by the Local Area Agency on
Aging, with approval for placement in the facility.
The majority of these requirements will be completed by the attending
physician or by our staff. However, should you need assistance in
